Our role is to help you understand your plan, connect with the right supports, and build the skills and confidence to manage your journey independently. Whether you’re just starting out or looking for ongoing guidance, we are here to help.
How we Support You
Compassionate guidance – Understand your NDIS plan and get the most out of your funding.
Personalised support – Research, identification and connection with local providers, community services, and programs suited to your goals and support needs.
Confidence and independence – Build the knowledge and skills to manage your supports effectively.
Coordination and management of supports – Set up and maintain service agreements, track your budget, and monitor the effectiveness of your supports.
Ongoing assistance – Get help preparing for Plan Reviews, resolving challenges, and exploring new options when things aren’t working.
Next Steps Pathway Planning provide both Level 1 (Support Connection) and Level 2 (Support Coordination), with flexible options available for short-term or ongoing support. We are able to work with both self managed and plan managed participants with Support Coordination included as a Capacity Building support on their plan.
